The total cost of the plan is $500 per year, and allows you to choose six visits from any of our providers. Services include:

Perfect for individuals or small businesses who want to provide their employees with health care benefits but can’t afford the cost.
Payment does not extend beyond one year from initial date and is due in full before any services are rendered. Prepaid health plan is non-transferable and patient must renew the prepaid health plan each year. Prepaid health plan does not include emergency room visits, in-hospital services, laboratory studies, or prescriptions.
